AYSO Volunteer Application Forms

To All Region 69 Referees:

If you have not personally (not your spouse) completed a Volunteer Application Form (either when you registered your child at Paul Revere in the Spring or at one of the Referee Meetings at Mort's), you need to do so ASAP. This is a National AYSO requirement and is important for insurance purposes.

Starting this evening (Friday), there will be a box of blank Forms at the home of James McCormick, which is located on the southwest corner of Sunset and Carey in the Palisades, across from the fire station. The box will be on the wooden fence to the left of the McCormick's driveway, which is on the Carey side of the house.

Once you have filled the form out completely, please mail it to:
