2008 Player Registration Procedures

• Pre-Registration must be done ONLINE. 

• As described on previous screens

• Mail or hand deliver the following to

  • 150 South Barrington Avenue # 5,

  • Los Angeles, CA 90049

  • (near Sunset, stairs next to Brentwood Bar & Grill)

  • Insufficient postage will result in delivery delay. 

  • No Express Mail, no "signature required" please!  This will delay delivery.

• ALL new players U8 and above must also be evaluated.

• Late Registrant Evaluations will be held at Brentwood Science Magnet – San Vicente/Gretna Green. Dates and times to be announced.

• Players should be dressed in shorts, soccer cleats and shin guards for assessment.

• Bring water as it is often hot on the field

1. SIGNED eAYSO Player Registration Form (2 copies per player)

2. SIGNED Region 69 Supplemental Info Form (2 copies per player!)

• Please staple each copy of (A) and (B) together (back to back)

3. SIGNED KID ZONE PLEDGE

4. Copy of PLAYER Birth certificate or Passport (for AYSO to keep)

5. Check for $195 (one player) $390 (two players), $585 (3 or more) (or Credit Card confirmation from eAYSO if used)

6. If Team Sponsor, check for $500 (one player), $750 (two players), $1,000 (3 or more)

7. SIGNED eAYSO Volunteer Application (2 copies per volunteer each set stapled back-to-back)

8. Copy of Volunteer's Driver's License

9. U16/U19 players - 2  photos, proof of age.
