Fall 2013 Registration Instructions

1. MAC users MUST use Internet Explorer, Netscape or Firefox

2. Read Welcome letter from our Regional Commissioner by clicking WELCOME

3. Read the Volunteer Position Descriptions by clicking DESCRIPTION. Decide which position would be best for you. YOU MUST CHOOSE ONE PER FAMILY

4. Read the Region 69 Registration Agreement by clicking AGREEMENT. You agree to this when you e-sign your Player REgistration Form on www.eAYSO.org. (Read Kid Zone Pledge)

5. Returning players may continue to register however waitlisting is possible after April 16th. The PRIORITY DEADLINE for Submitting RETURNING Player Applications and payment was April 16th.  

6. NEW PLAYER REGISTRATION will be at Paul Revere Middle School

7. You MUST FIRST register online and then bring eAYSO Player Registration Form, at least one eAYSO Volunteer Form, proof of online payment (or check), and proof of age to the Registration/Evaluation.  DO NOT MAIL FORMS.

8. The Registration Fee is $250. PAYMENT may be made online by credit card on www.eAYSO.org, or by check with player name clearly indicated.  

9. Registration involves 1 ONLINE step ONLY for Players AND Volunteers.   (Site is down most Monday evenings 6pm-Midnight)

• For First Time Parents   (Read instructions then click on www.eayso.org (New window opens)

  1. New Families:  Click FIRST TIME and follow the prompts to set up your eAYSO account.  Remember your login and password.

  2. Your Region is 69. Enter or select this where requested

  3. Next few eAYSO screens are informational.  You’ll set up your account and then complete an application for your first player. 

  4. Print 1 copy of this form.  You’ll have the option to pay by credit card (print the cc confirmation sheet), or you may pay by check.

  5. Once you have set up your account, and completed one application, you’ll see the various options on the welcome page.  You can click MYeAYSO if you don’t see the options.

  6. PARENTS JOBS - At least one parent must volunteer for a position even if "OTHER".  If you want to COACH, ASST COACH, or REFEREE, you should so indicate.   

  7. For payment, you may send a check, or you may use the credit card option.

  8. You may now sign up a 2nd player,  by clicking on application for a new player in left column.

  9. Click on Volunteer . At least one parent from each family must submit a Volunteer Form online even if "OTHER". If you want to COACH, ASST COACH, or REFEREE, you should so indicate on this form.    Look in the right column to also submit youth volunteer forms for your children, if they are volunteering to referee or VIP buddy.  

  10. Click on Print Form and print 1 copy of each Volunteer form you have filled out and sign using BLUE ink.

   

  What to do with all these forms?

  • Bring the Player Registration form, the Volunteer form, payment (or proof of online payment, and a copy of a birth certificate or passport to the onsite Registration/ Evaluation session to be held at Paul Revere Middle School

  • Players 6 years of age and older as of 7/31/2013 must attend the in person registration day to be evaluated.  If they are unable to attend, they will still need to be evaluated. Check website after June 1st for other dates.

   

  Check List:

  • 1 copy of the eAYSO Player Registration Form for each player.

  • 1 signed copy of the Volunteer form for at least one parent.

    - If form printed on 2 pages, please staple back to back. 

  • 1 signed copy of Youth Volunteer form for each youth volunteer.

  • Check  or credit card payment (include confirmation) -

    • $250

  • Copy of proof of age, either passport or birth certificate.

  • U16 & U19 PLAYERS ONLY:  Copy of Birth Certificate and 2 one inch COLOR photos REQUIRED.

eAYSO Instructions for RETURNING Families  - Read instructions then click on www.eayso.org (New window opens)

1. Returning Families:  Your login is the e-mail address you used last season. Please DO NOT set up a new account. 

   • The site provides help if you forgot your login or password. For local help, contact registration69@aol.com

   You MUST update and electronically sign and submit all forms, for both players and volunteers—EACH YEAR. 

   • One signed ("wet signature") copy of the player registration form and the Kid Zone pledge must be provided to the coach prior to the season beginning in September.  

2. Click on Volunteer (New or Continuing). At least one parent from each family must submit a Volunteer Form online even if "OTHER". If you want to COACH, ASST COACH, or REFEREE, you should so indicate oin this form.    Look in the right column also submit youth volunteer forms for your children, if they are volunteering to ref, coach or VIP buddy.  

3. * new Adult Volunteer ONLY:  Click on Print Form and print 1 copy of the form. Mail this form and a copy of your driver’s license to the office ASAP.

4. Click on APPLICATION for a returning player, or new (sibling) player,  in the left column.

5. Verify or enter your child’s birthday and other information.  Do this for each child applying.

6. Make sure BOTH parents have been entered with their CURRENT email addresses.

7. PARENTS JOBS - At least one parent must volunteer for a position even if "OTHER".  If you want to COACH, ASST COACH, or REFEREE, you should so indicate.   

8. For payment, you may send a check, or you may use the credit card option while activated on the website. (If it is not active, only payments by check are accepted) 

9. Returning players may continue to register however waitlisting is possible after April 16th.

10. U16 & U19 PLAYERS ONLY:  Copy of Passport or Birth Certificate and 2 one inch COLOR photos REQUIRED. Mail this to the office ASAP.

• Online Volunteer form for at least one parent per family plus any other volunteers.

  - If each form printed on 2 pages, staple back to back.

• Online Youth Volunteer form for each youth volunteer.

• Check  or credit card payment (include confirmation)

  • $250

• U16 & U19 PLAYERS ONLY:  Copy of Birth Certificate and 2 one inch COLOR photos REQUIRED.

• Your credit card confirmation or your cancelled check (by 6/1) is notification of successful registration. 

AYSO Youth Volunteers

1. If you did not previously perform the registration process above, you can do this by going to eAYSO, log in with family login and click on My eAYSO.

2. Proceed with this step if your child is volunteering for youth referee, coach, VIP buddy or other positions.

3. Click on the appropriate Youth Volunteer application (i.e. New or Returning) in the right column.

4. Print 1 copy and sign in BLUE.

5. Return these forms by MAIL to:
