• Player Registration (Medical Release)

Bring signed copies plus the following to 1st tryout

• A copy of player’s birth certificate or passport to leave.

• Two 1” x 1” up to 1 ¼” x 1 ¼” current color photos for ID card to leave.

• 2 checks made payable to Region 69

  • 1 for $450 (Team fee)

  • 1 for $150 (Region fee)

• Cleats, shorts, socks, shirt, water

PLAYERS WILL BE CONTACTED AFTER THEIR DIVISION CHAMPIONSHIP (Dec 12th-18th)
