All Star Application

AYSO REGION 69 - ALL STAR PLAYER APPLICATION 2007-2008

Enclose the following in an envelope and bring to the first tryout attended.

1. Completed Player Application. Please list any questions or concerns on the back of this sheet.

2. Completed Player Registration Form (Medical Release) DOWNLOAD HERE

3. A copy of player’s birth certificate or passport to leave.

4. TWO 1” x 1” up to 1 ¼” x 1 ¼” current color photo for ID card - to leave.

5. 2 checks made payable to Region 69 - 1 for $450 (Team fee) 1 for $150 (Region fee).

These will not be deposited until after selections have been made. Checks will be returned to those not selected.
